BS KIDS MEMBERSHIP AGREEMENT
Thank you for your interest in BS Kids and in our membership packages. Below you will find our
Contract Agreement, depending on the package that you choose. At this point, you should have filled out
the Information Log, which details your present situation. Remember you need to bring your payment
with you, in addition to the contract form during your initial meeting. The agreements below simply
outline the Membership Packages and should be signed either prior to our first meeting or at the first
initial consultation. As always, if you have any questions, let us know at help@bskids.net.

SILVER PACKAGE MEMBERSHIP CONTRACT
BS Kids LLC and _________________________ on ____________ , ____ of 2008 enter into an
agreement for services regarding Family/Domestic Issues. This agreement begins on __________, __,
2009 and will expire on _____________, __ of 2010.
This agreement is for services and information relating to ______________, which the above party has
contracted with BS Kids LLC. BS Kids LLC agrees to provide services and information, specified in the
Silver Package at five hours of legal research and information for $ 425.00. If the hours required exceeds
the above mentioned allotment, an addendum will be entered into between both parties. This will state the
additional hours and fees associated with these charges, to be billed at
$70.00 per hour, per the Silver Membership package. Checks are to be made payable to BS Kids LLC.
BS Kids LLC is not a substitute for legal advice. We strongly recommend that you seek the advice of an
attorney, one the information and services is provided to you by BS Kids LLC. BS Kids LLC mission is to
provide you with the information you need, in order to move forward with your current dispute.
BS Kids LLC does not charge for the initial appointment. If after your initial meeting, both parties agree that
BS Kids LLC can not meet your current needs, there is no required payment required. Payment for the
Silver Membership Package is due at the initial meeting, providing both parties agree to move forward. If at
any time you are dissatisfied with the service, once BS Kids LLC has begun to research your issue, you
may cancel your membership with a $ 75.00 cancellation fee. Failure to pay on a timely basis will result in
termination of services and further attempts for payment will be handled by a collection agency.
Payment arrangements can be made as long as both parties are in agreement and any changes in
payments, not following with this agreement, must be submitted in writing and signed by both parties.
I hereby agree to accept the terms and conditions and enter into an agreement with BS Kids LLC for
research and informational services regarding my pending dispute. I agree to pay, upon remittance and
understand that this is not legal advice.
